Need to locate health care and community resources? Having difficulty navigating the health care system? The Caregiver Support Program provides integrated information that brings people and services together in the community of Vancouver. We help caregivers make informed choices regarding their particular needs, and the services that might be helpful to them. The program is regularly contacted by caregivers, community partners, and health professionals seeking information about the following:
- Access to disease, illness or disability specific groups and information
- Navigating the health care system and how to connect with community health services (Long Term Care)
- Facility Placement (Nursing Home placement)
- Home Support guidelines and services offered
- Caregiver support groups
- Seniors' programs
- Community resources
